The Case Manager – Support Services works with transition-aged youth to identify goals and develop service plans that drive housing stability and emotional wellness.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Counseling, Education or related field.
• Minimum two years of experience with transition-aged youth, foster youth, housing programs, shelters, or community-based services.
• Strong understanding of trauma, attachment, adverse childhood experiences, and the transition from foster care to adulthood.

Responsibilities

• Provide direct, trauma-informed support to transition-aged youth and young adults with histories of foster care, housing instability, and complex trauma.
• Collaborate with participants to identify goals and develop individualized service plans.
• Coordinate access to housing resources, transportation, education, employment, healthcare, and other community-based supports.
