The Case Manager – Support Services serves as a key member of the I Am Next support services team and provides direct, trauma-informed support to transition-aged youth and young adults with histories of foster care, housing instability, and complex trauma.
This position works collaboratively with participants to identify goals, develop individualized service plans, and support progress toward housing stability, self-sufficiency, emotional wellness, and long-term independence. The Case Manager provides relational support while coordinating access to housing resources, transportation, education, employment, healthcare, behavioral health services, and other community-based supports.
Using strengths-based, motivational, and trauma-informed approaches, the Case Manager empowers young adults to navigate barriers, build resiliency, and develop the skills necessary for successful adulthood.
