Job summary

CHNK Behavioral Health in Williamstown, KY is seeking a Targeted Case Manager (OneQuest Health) to support clients and families in accessing behavioral health and related services. You will advocate, coordinate with the Outpatient team, and connect clients with local resources.

Qualifications

• BS or BA in social work or in a related behavioral science field with a minimum of one year full-time experience or be a certified alcohol and drug counselor. A Master’s degree may substitute for the one year of experience.
• Must successfully complete case management training within 6 months of hire and annual continuing education requirements.
• Organizational skills and basic computer skills.
• Must have a current driver’s license and driver’s insurance.
• Demonstrates ability to work as an effective member of a multi-disciplined team.
• Demonstrate proven sensitivity to the various cultural and socioeconomic characteristics of Children’s Home clients and staff.

Responsibilities

• Assist clients and families in accessing behavioral health treatment and related services.
• Advocate for clients and collaborate with Outpatient clinical team and community partners.
• Link clients to local resources and enhance social and emotional functioning.
