WestCoast Children's Clinic in Oakland, CA seeks a Case Manager and Engagement Specialist to empower foster and transition-age youth through clinical case management, outreach, and trauma-informed care. The role supports two programs (STAT and AWOL) and requires building trust, coordinating care, and navigating Medi-Cal with families.

The position is Regular Full Time, hybrid in Oakland, with full-time weekday hours and bilingual Spanish candidates earning a higher rate.

Qualifications

• Minimum of two years of experience working with systems-involved youth.
• Bachelor's degree in a relevant field preferred.
• Bilingual in English and Spanish preferred.

Responsabilités

• Deliver individualized clinical case management, rehabilitation, and flexible care via community outreach and telehealth.
• Support youth experiencing runaway behavior or placement disruptions, advocating for safe transitions into foster, relative, or group homes.
• Provide targeted early interventions to stabilize crises (STAT) and address placement challenges.
• Empower youth to engage in care planning while connecting them to mental health services, mentorship, and community resources.
• Bridge service gaps for youth missing from care to address physical, emotional, and safety needs.
• Assist youth with navigating Medi-Cal coverage to ensure continuous healthcare and therapeutic support.
• Offer trauma psycho-education and advocacy to help youth process experiences and build self-advocacy skills.
• Guide youth through delicate placement transitions with emotional and practical support.
