Job summary

Boys Town Louisiana, Inc. is seeking a Care Coordination Services Consultant in New Orleans to guide youth and families through complex care systems. You will provide intensive case management and connect families to essential resources, advocating for long-term stability and well-being.

The role is full-time, with a schedule that includes non-traditional hours and rotational crisis response. A Bachelor's in Human Services plus 1+ year experience is required, along with a valid driver's license

Qualifications

• Bachelor's degree in Human Services or related field required.
• 1+ year of experience working with children and families.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Provide strengths-based, trauma-informed case management services.
• Coordinate community resources for youth and families across systems.
• Support families in navigating child welfare, mental health, education, and justice systems.
• Monitor service plans, provide crisis intervention, and promote permanency planning.
• Maintain accurate documentation and regulatory compliance.
