Job summary

WHITE MOUNTAIN APACHE BEHAVIORAL HEALTH SERVICES, INC. seeks a Thrive Case Manager to coordinate behavioral health services for clients across clinic, community, and home settings. Responsibilities include transportation, outreach, and collaboration with schools and tribal partners.

Responsibilities also cover documentation, treatment plan updates, and ensuring AHCCCS/ABHS standards are met while advocating for clients with diverse needs and trauma-informed care.

Qualifications

• High school diploma or equivalent with 18 college credits in behavioral health, or 4 years of related work experience.
• Associate/Bachelor’s degree in mental health/human services or related field preferred; Native American experience preferred.
• Excellent written and oral communication skills; Apache language ability preferred.
• Demonstrate critical thinking, problem-solving, and organizational/time management skills.
• Strong interpersonal skills for teamwork with diverse groups.
• MS Office proficiency and ability to operate copy machine and fax.
• Valid Arizona Driver’s License and reliable transportation.
• Ability to pass pre-employment testing including fingerprint clearance, drug test and background check.

Responsibilities

• Manage a caseload of clients who may benefit from behavioral health services.
• Provide support to ABHS clinical staff including transportation and coordination.
• Facilitate group counseling, skill training, and education programs.
• Document client contacts, assess needs, and update treatment plans per AHCCCS/ABHS standards.
• Coordinate services between clients, families, and community partners.
• Engage in client outreach and re-engagement in treatment services.
• Provide client transportation to services as needed.
• Support clinical staff with attendance at interdepartmental meetings.
