• NIH Stroke Scale certification and 8 hours of Stroke-related CNE annually if assigned to a related unit.

Responsibilities

• Plan and provide individualized comfort measures and communicate with the health team.
• Follow up on internal resources to determine outcome of interventions.
• Collaborate with case manager to discuss alternatives to care.
• Educate patients and families about health conditions and management.
• Develop and document a discharge plan to maximize patient recovery.
