Job summary

Fox Point Recruitment LLC seeks a Manager of Case Management (Trauma) in Houston, TX to lead inpatient case management, ensure coordinated care, utilization management, and discharge planning. You will partner with leaders to improve outcomes and resource use.

The ideal candidate has a nursing or social work background with hospital case management, utilization management, and leadership experience. Attractive relocation and a no visa sponsorship stance are noted.

Qualifications

• BSN or BSW required.
• Current Texas RN or LMSW license.
• Case Manager Certification required.
• Minimum five years in utilization/case management or discharge planning.
• Minimum three years in hospital-based nursing or social work.
• Minimum three years in leadership roles.

Responsibilities

• Lead inpatient case management program and daily operations.
• Coordinate care facilitation, utilization management, discharge planning.
• Drive quality improvement and performance metrics.
• Collaborate with physicians, nursing leadership, social workers, and care coordinators.
• Recruit, mentor, and evaluate staff; manage a multidisciplinary team.
• Utilize data to implement performance improvement strategies.
• Ensure compliance with policies and regulatory standards.
