Job Summary

Responsible for providing safe transportation to patients and guests in a timely, competent manner and communicating important information relevant to the overall care plan to the multidisciplinary team.

Job Duties

1. Responsible for van operation and escorting passengers during transport to and from pick-up and delivery sites.
2. Follow established schedules, make adjustments for unanticipated changes, determine the safest and most efficient routes considering participants' needs and clinic requirements, and maintain a narrative of routes.
3. Safely perform one-person transfers of passengers to and from vehicles and inside vehicles, ensuring all passengers wear safety belts and wheelchairs are locked into position.
4. Promptly report any patient or guest-related concerns observed during home or van transport.
5. Document incidents, accidents, and other relevant information according to program policy.
6. Inspect vehicles regularly and maintain them in a clean and safe condition.
7. Maintain daily logs of passenger information, mileage, service records, etc.
8. Provide support services to other departments as needed, such as transporting documents to post office, bank, etc.

Work is typically performed in a transportation vehicle or office. Must be able to lift at least 50 pounds, sit and drive for extended periods, and comply with all organizational policies and procedures.
