Responsibilities

• Coordinate transportation schedules with nursing staff and outside providers.
• Communicate clearly regarding resident needs, schedule changes, and condition updates.
• Complete pre-trip vehicle inspections and report safety concerns.
• Maintain cleanliness and safety standards for transportation vehicles.
• Follow facility policies, state regulations, and infection control guidelines.
• Provide supportive CNA care as trained.

This position plays an important role in ensuring residents are safely transported to and from medical appointments and approved outings while providing respectful, supportive care throughout the process.

• Must complete and maintain required physician-signed Vehicle Driver Fitness for Duty documentation
• Ability to safely assist residents with mobility, including transfers and wheelchair support
• Strong organizational and time-management skills

• Compassionate, patient-centered approach to resident care

Responsibilities

• Safely transport residents to and from medical appointments, community outings, and other approved destinations
• Provide hands-on assistance with resident transfers in and out of the facility, vehicle, and appointment locations

• Coordinate and manage transportation schedules in collaboration with nursing staff and outside providers
