Job summary

CareCore at Lima is seeking a dependable, organized, and resident-focused Transportation Coordinator to join our team. This role safely transports residents to medical appointments while coordinating transportation needs.

Typical schedule: Monday-Friday, 9:00 AM-5:00 PM, with some weekend availability based on resident appointments and facility needs. You will engage with residents, families, nursing staff, and leadership to ensure safe, timely transport and exceptional service.

Qualifications

• Must be at least 25 years of age to operate the company vehicle.
• Valid driver’s license and acceptable driving record required.
• STNA/CNA or DSP experience strongly preferred.
• Experience with seniors or in a healthcare setting is a plus.
• Strong communication and organizational skills.

Responsibilities

• Safely transport residents to and from medical appointments and other approved destinations.
• Coordinate and maintain resident transportation schedules.
• Communicate appointment and transportation information with residents, families, nursing staff, and facility leadership.
• Assist residents with safely entering and exiting the company vehicle.
• Ensure the company vehicle is kept clean, safe, and ready for use.
• Maintain transportation documentation and records.
• Provide excellent customer service and compassionate support to residents.
• Assist with additional duties within the facility during downtime when no transports are scheduled, as assigned by the Administrator.
• Follow all facility policies, safety procedures, and resident confidentiality requirements.

• Must be available for occasional weekend transportation needs as required
