Northern Manhattan Rehabilitation & Nursing Center in Manhattan seeks a Transportation Certified Nursing Assistant (CNA) to support residents with appointments and transfers. Part-time schedule, 7:00 AM–3:00 PM, Monday–Friday, with walk-in interviews welcomed.

Active NY CNA certificate and NY Driver’s License required; at least 3 months’ CNA experience in subacute care or a nursing home. You will accompany residents, assist transfers, and coordinate communication with the facility.

Qualifications

• Active New York Certified Nursing Assistant (CNA) certificate.
• Valid New York Driver’s License.
• At least 3 months of CNA experience in subacute care or a nursing home (clinical experience while going through a CNA program doesn’t count).
• Must be willing to work on the CNA floor when needed.

Responsabilités

• Accompany residents to medical appointments outside the facility.
• Safely assist residents with transfers, including wheelchair and stretcher transfers.
• Work on the CNA floor when needed.
• Serve as a liaison between residents and the facility by providing and obtaining information as needed.
• Ensure appropriate communication and follow-up after resident appointments.
• Hand-carry required documents, including medical records and forms, as well as specimens.
• Complete additional duties as assigned by the Appointment Coordinator and/or Director of Nursing.
