Job summary

Northern Manhattan Rehabilitation & Nursing Center is seeking a Transportation Certified Nursing Assistant (CNA) for a part-time schedule in Manhattan. You will accompany residents to medical appointments, assist with transfers, and support on the CNA floor as needed.

Join a diverse team focused on resident care and communication. The role requires NY CNA certification, a valid NY driver’s license, and at least 3 months of CNA experience in subacute or long-term care.

Qualifications

• Active New York Certified Nursing Assistant (CNA) certificate.
• Valid New York Driver’s License.
• At least 3 months CNA experience in subacute care or a nursing home.
• Must be willing to work on the CNA floor when needed.

Responsibilities

• Accompany residents to medical appointments outside the facility
• Safely assist residents with transfers, including wheelchair and stretcher transfers
• Work on the CNA floor when needed
• Serve as a liaison between residents and the facility by providing and obtaining information as needed
• Ensure appropriate communication and follow-up after resident appointments
• Hand-carry required documents, including medical records and forms, as well as specimens
• Complete additional duties as assigned by the Appointment Coordinator and/or Director of Nursing
