Northern Manhattan Rehabilitation & Nursing Center is hiring a Transportation Certified Nursing Assistant (CNA) for a part‑time schedule in a Manhattan location. The role involves accompanying residents to appointments, assisting with transfers, and serving as a liaison for medical information while on the CNA floor when needed.

Qualifications

• Active New York Certified Nursing Assistant (CNA) certificate.
• Valid New York Driver’s License.
• At least 3 months of CNA experience in subacute care or a nursing home.

Responsibilities

• Accompany residents to medical appointments outside the facility.
• Safely assist residents with transfers, including wheelchair and stretcher transfers.
• Work on the CNA floor when needed.
• Serve as a liaison between residents and the facility by providing and obtaining information as needed.
• Ensure appropriate communication and follow-up after resident appointments.
• Hand-carry required documents, including medical records and forms, as well as specimens.
• Complete additional duties as assigned by the Appointment Coordinator and/or Director of Nursing.
