Job summary

NeighborHealth seeks a Transportation Associate to support health center PACE services for frail elderly patients and provide courier services between NeighborHealth sites.

You will transport participants to appointments, assist with mobility using a lift or stairs, and ensure safety by securing all passengers and equipment. This onsite role requires a valid MA driver’s license and a three-year clean driving record.

Qualifications

• HS Diploma/GED required.
• Current MA driver’s license.
• Safe driving record for three years.

Responsibilities

• Provides transportation services to frail elderly patients and families.
• Transports participants to/from home to PACE Center or appointment using lift, wheelchair, or walking assistance.
• Secures participants and equipment in the vehicle and adheres to transportation requirements.
• Maintains and operates vehicles and equipment per EBNHC requirements.
• Collaborates with intake/assessment team for participant evaluation and re-evaluation.
• Adheres to infection control procedures and provides courier service between NeighborHealth locations and PACE sites.
