Job summary

CommuniCare Family of Companies’ Maplewood Healthcare Center in Bridgeport, WV is seeking a Transportation Assistant. You will transport residents to outside physician visits and within the facility, with duties that may include activities dept. and CNA tasks as needed.

The role emphasizes safety, resident care, documentation, and collaboration with a care team. Dayshift, Monday–Friday with weekends as needed, with opportunities to grow within a supportive post-acute care system.

Qualifications

• High school diploma or GED.
• Valid WV Driver's License and WV CNA License.
• Graduate of an approved Nurse Aide Training Program or be eligible to enter such a program and graduate and pass a state test within 120 days from date of employment.
• Must possess Red Cross First Aid Certificate.
• Compassionate and enthusiastic personality.

Responsibilities

• Transport and assist residents to outside physician visits in a safe manner.
• When not in transportation, work in Activities department and as CNA if needed.
• Transport to scheduled appointments (podiatry, audiology, optometry, dental, etc).
• Transport residents to in-house activities.
• Provide one-on-one monitoring for elopement risk during outside visits.
• Perform basic nursing care procedures as required.
• Create a clean, comfortable and safe environment and meet residents' needs.
• Maintain and update resident documentation records.
• Follow established policies, procedures, safety and quality standards and participate in training.
• Perform other related activities as assigned.

Maplewood Healthcare Center, a member of the CommuniCare Family of Companies is currently recruiting for a Transportation Assistant to join our team. Primarily Dayshift, Monday - Friday with weekends as needed.

PURPOSE/BELIEF STATEMENT:

The position of Transportation Assistant exists to assist residents with transportation to and from inside facility and outside physician appointments and events in a safe manner and in accordance with established procedures. This position must work effectively with team members in ensuring that work is accomplished and quality care is delivered, supporting their team members and celebrating team successes. While focusing on delivering quality resident care, the position must also manage the resources that are available to them in their job.

JOB DUTIES & RESPONSIBILITIES

• Transport and assist residents, per facility vehicle, to outside physician visits in a safe manner.
• When not working in transportation, will be expected to work in Activities department as well as working as a CNA if needed.
• Assist with transporting residents within the facility to scheduled specialty appointments, i.e. podiatry, audiology, optometry, dental, etc.
• Assist with transporting residents to and from scheduled in-house activities.
• Provide one on one monitoring for residents deemed an elopement risk on outside visits.
• Perform basic nursing care procedures for residents as required.
• Create a clean, comfortable, and safe environment for residents and assure that their needs are met.
• Maintain resident documentation records.
• Maintain and work within established departmental, center, and corporate policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards.
• Attend and participate in scheduled training, educational classes, and orientation programs to maintain certification and enhance quality of care.
• Perform other related activities as assigned or requested.

QUALIFICATIONS & EXPERIENCE REQUIREMENTS

• High school graduate or G. E. D. equivalent
• Valid WV Driver's License & WV CNA License Required
• Graduate of an approved Nurse Aide Training Program or be eligible to enter such a program and graduate and pass a state test within 120 days from date of employment.
• Must possess Red Cross First Aid Certificate.
• Prior work/life experiences involving independent decision making
• Prior work/life experiences, preferably in healthcare or a service industry
