Job summary

Maplewood Healthcare Center in Bridgeport, WV is seeking a Transportation Assistant to help residents travel to and from doctor visits and activities, with duties also spanning the Activities department and CNA coverage as needed.

The role requires a WV Driver's License, WV CNA License, and completion of a Nurse Aide Training program or eligibility, plus Red Cross First Aid certification. Shift is primarily days, Monday–Friday, with weekends as needed.

Responsibilities

• Transport residents to outside physician visits in a safe manner.

• Work as CNA when transportation duties are light.
• Monitor residents deemed elopement risks during outside visits.
• Provide basic nursing care procedures as required.
• Maintain resident documentation and records.

Job Address:1081 Maplewood Drive Bridgeport, WV 26330Maplewood Healthcare Center, a member of the CommuniCare Family of Companies is currently recruiting for a Transportation Assistant to join our team.Primarily Dayshift, Monday - Friday with weekends as needed.PURPOSE/BELIEF STATEMENT:The position of Transportation Assistant exists to assist residents with transportation to and from inside facility and outside physician appointments and events in a safe manner and in accordance with established procedures. This position must work effectively with team members in ensuring that work is accomplished and quality care is delivered, supporting their team members and celebrating team successes. While focusing on delivering quality resident care, the position must also manage the resources that are available to them in their job.JOB DUTIES & RESPONSIBILITIESTransport and assist residents, per facility vehicle, to outside physician visits in a safe manner.When not working in transportation, will be expected to work in Activities department as well as working as a CNA if needed.Assist with transporting residents within the facility to scheduled specialty appointments, i.e. podiatry, audiology, optometry, dental, etc.Assist with transporting residents to and from scheduled in-house activities.Provide one on one monitoring for residents deemed an elopement risk on outside visits.Perform basic nursing care procedures for residents as required.Create a clean, comfortable, and safe environment for residents and assure that their needs are met.Maintain resident documentation records.Maintain and work within established departmental, center, and corporate policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards.Attend and participate in scheduled training, educational classes, and orientation programs to maintain certification and enhance quality of care.Perform other related activities as assigned or requested.QUALIFICATIONS & EXPERIENCE REQUIREMENTSHigh school graduate or G.
