Job summary

Responsibilities

  • Transport clients to and from agency services in vans in a safe and timely manner
  • Adhere to priority transportation schedule and adjust as needed
  • Maintain and document vehicle maintenance, fueling, and repairs
  • Keep vehicle mileage log and monitor license/registration and inspections
  • Assist in office duties when not transporting, including answering phones and scheduling
  • Establish and maintain filing system for client documentation, records and messages
  • Input data into databases accurately and timely, and protect client confidentiality
  • Attend staff meetings and training sessions and contribute to service delivery

Job description

To provide transportation services for clients of Special Health Resources. Position may also be responsible for providing deliveries of supplies for agency and/or clients. Also required to provide general maintenance for vehicles. Responsible for safety of clients receiving transportation services. Responsible for seeing that maintenance, repairs, and cleanup are performed on van. Position requires good independent judgment skills. Ability to reassess schedule and adjust on a moments notice. Documentation abilities. Ability to lift and carry 5O lbs. of weight five feet. Oral and written communication skills.Primary responsibilities are typically; organizing and maintaining the filing systems for medical services and medical records, support staff for answering telephone, scheduling appointments, providing the public general information, and taking/delivering messages. Other significant responsibilities may include preparing correspondence, checking and sorting the mail, imputing information into databasesQUALIFICATIONS:Requires a valid Texas Driver's License, appropriate driving record and auto liability insurance that meets the minimum required by State of Texas statute.Mental and physical health sufficient to meet the demands of the position.Requires one (1) year experience in the public transportation field or one (1) year experience as a direct care provider in human services field.ESSENTIAL FUNCTIONS INCLUDE BUT NOT LIMITED TO:Transport clients to and from services of the agency in the agency vans in a safe and timely mannerAdhere to priority schedule for transportation, when appropriateTransport Mobile Unit to different sites when neededKeep transportation up to date, and document services in client fileResponsible for all scheduled maintenance, needed repairs, fueling, and documentation of all of theseHelping to monitor vehicle license (registration) and inspectionKeep a vehicle mileage log formReport any safety concern or otherwise, to Regional CoordinatorWork in close contact with team to ensure proper transportation to and from servicesAssist in the office when not transporting, to help with phones, client needs, etc.Establish and maintain a filing system for documentation of clients receiving medical and case management services.Records client sensitive information by approved methods.Maintains client confidentiality at all times.Maintains effective filing system and files necessary information in a timely manner.Identifies areas for improvement and implements themCommunicates details regarding all systems and changes with staff membersPrepares confidential and non-confidential correspondence in a professional style and format.Inputs data into databases correctly and in a timely manner.Makes appointments as needed and assists staff with scheduling/coordination.Relieves other clerical staff as needed.Relays information to the public as needed.Attend staff meetings, both programmatic and agency wideAttend appropriate training for position, and continuing education provided by the agencyServe as customer service representative with clients and others while away from the office providing transportationOther duties as assigned
