Qualifications

• Completed CNA Course/State Licensed preferred.
• Medical Records experience in long-term care setting preferred.

Responsibilities

• Schedule and/or provide resident transportation to/from appointments.
• Assist with daily activities, resident safety, and personal care.
• Comply with all documentation/record-keeping policies including vitals.

Responsibilities:

• Schedule and/ or provide resident transportation to/ from appointments

• Resident care according to an approved Care Plan

• Assist with daily activities, resident safety, and personal care

• Understand and practice infection control procedures

• Comply with all documentation/record-keeping policies including vitals

• Reports all violation of the Residents’ Bill of Rights immediately

QUALIFICATIONS:

• Completed CNA Course/State Licensed (Preferred, not Required)

• Clean driving record

• High School diploma or GED

• Medical Records experience in long term care setting, preferred

• ART, preferred

• Excellent communication skills; written and verbal

• Service mentality and a thirst for learning

• Positive Can-Do Attitude

• Excellent Attendance
