Job summary

Froedtert Health in Milwaukee, WI is seeking a Transplant Social Worker for our Froedtert Hospital campus. This onsite, full-time role supports patients across Nephrology, Advanced Heart Failure, VAD and Solid Organ Transplant programs, providing psychosocial assessment and care coordination.

The role requires a Master’s Degree in Social Work, current Wisconsin social work licensure, and a minimum of two years in health care.

Qualifications

• MSW with medical social work experience is preferred.
• WI social work licensure is required.
• Minimum 2 years experience in health care is required.

Responsibilities

• Act as liaison between families, hospital staff and patients across nephrology, heart failure and transplant services.
• Plan, implement and facilitate social service interventions for transplant patients.
• Provide psychosocial assessment and ongoing updates to address patient needs and access resources for ventricular assist devices or transplants.
