Job summary

Virtua Health in New Jersey seeks a Social Worker specialized in transplant and advanced organ disease to provide psychosocial support and navigation for patients, living donors, and caregivers across inpatient and outpatient settings.

You will collaborate with a multidisciplinary team, conduct comprehensive assessments, and ensure regulatory documentation while offering crisis counseling and community referrals as needed.

Qualifications

• Two to three years of healthcare experience focusing on transplant or advanced organ disease.
• Strong communication, discharge planning, and care coordination skills.
• Ability to manage a cumulative caseload across inpatient/outpatient settings.

Responsibilities

• Inpatient and outpatient psychosocial assessment and counseling across care phases.
• Coordinate follow-up and develop discharge plans with the care team.
• Provide crisis intervention, referrals, and education to patients and families.
• Document all assessments and interventions per regulatory standards.
• Participate in multidisciplinary rounds and program development to enhance transplant services.
