Job summary

Banner Health seeks a Registered Nurse (RN) Heart Transplant Coordinator to join the Heart Transplant team at Banner University Medical Center in Phoenix. This day shift role runs Monday through Friday, 7:00 AM to 4:30 PM, with on-call rotations including weekends and holidays.

You will manage pre- and post-heart transplant patient care, collaborating with the multidisciplinary team to support the transplant journey and improve outcomes.

Qualifications

• Current RN license in state of practice
• BLS certification required
• Minimum of two years clinical nursing experience
• Clinical proficiency in transplant patient care and care coordination
• Strong oral and written communication and time management skills

Responsibilities

• Manage care for pre- and post-transplant patients across the care continuum
• Coordinate evaluations, donor processes and post-transplant follow-up with the MDT
• Facilitate organ allocation and admission planning in collaboration with transplant teams
• Educate patients, families and payors on transplant processes and programs
