Job summary

Banner Health is seeking an RN Heart Transplant Coordinator to join the Heart Transplant team at Banner University Medical Center (BUMC) Phoenix. This day shift role runs Monday through Friday, 7:00 AM to 4:30 PM with some schedule flexibility and 24-hour on-call rotations including weekends and holidays.

The coordinator will care for pre- and post-heart transplant patients, collaborating closely with a multidisciplinary team.

Qualifications

• Requires current RN license in state of practice.
• BLS certification is required.
• Minimum of two years clinical nursing experience.
• Bachelor's degree preferred.
• Ability to work effectively in an interdisciplinary team.

Responsibilities

• Manages care coordination across the transplant care continuum.
• Coordinates pre-transplant evaluations and post-transplant follow-up.
• Works with donor evaluation, scheduling, and education of patients/families.
• Interacts with payors and ensures authorization for services.
• Communicates with transplant physicians, care teams, and patients to implement care plans.
