Job summary

Virginia Mason Franciscan Health in Seattle seeks a Transplant Coordinator RN to oversee renal and pancreas transplant coordination and support daily transplant operations.

You will act as case manager from referral through evaluation, inpatient stay, and post-transplant transition, drawing on your RN transplant experience and strong collaborative skills to guide patients and families across multiple states and care teams.

Qualifications

• Must hold WA State Registered Nursing License at hire.
• Registered Nurse in WA at hire.

Responsibilities

• Pre-Transplant Recipient Coordination: Educate patients and caregivers across multiple states and coordinate evaluations and tests.
• Living Donor Coordination: Manage pre- and post-donation activities and provide donor education and support.
• Post-Transplant Recipient Coordination: Conduct rounds, discharge planning, and transitions of care.
• Support patient and family experiences with multidisciplinary groups pre and post-operatively.
• Facilitate education classes for transplant recipients and families.
• Conduct outreach and education to referring providers and medical teams.
