Job summary

CommonSpirit Health's Baylor St. Luke's Medical Center in Houston is seeking an RN Transplant Coordinator to orchestrate the transplant journey from referral to recovery.

You will manage referrals, evaluate candidates, lead in-hospital care, coordinate education, and communicate with payers and all teams to ensure data-driven outcomes. You will drive education initiatives, collaborate with clinicians and researchers, and maintain comprehensive records to support program excellence and patient

Qualifications

• BSN required with 3+ years of experience at hire
• Experience in Acute Care/ICU, dialysis or transplant
• RN license in TX at hire; BLS CPR within 14 days

Responsibilities

• Review medical information, schedule testing, review results and compile data for presentation of patient's case.
• Notify consultants, prepare for and participate in rounds, and communicate with patients, families and all disciplines involved in patient management to streamline problems.
• Maintain effective communication with all stakeholders.
• Initiate and oversee pre-op and post-op education with nursing staff; assess learning barriers.
• Initiate and collaborate in research protocols, journal articles, abstracts and patient education material.
• Screen potential donor calls; coordinate donor/recipient status and manage 24-hour donor call responsibilities.

Job Summary and Responsibilities

As an RN Transplant Coordinator, you will be a pivotal force in transforming lives, orchestrating the intricate journey of transplant patients from referral to recovery.

Every day, you will expertly manage the full spectrum of transplant coordination: from initiating referrals and evaluating candidates, to leading in-hospital patient care, managing education, facilitating communication with all stakeholders including payers, and maintaining critical data.

To be successful in this role, you will combine advanced clinical expertise with exceptional organizational and communication skills, a meticulous eye for detail, and an-unwavering commitment to patient advocacy, driving both individual success and program excellence.

• Review medical information, schedule testing, review results and compile data for presentation of patient's case.
• Notify consultants, prepare for and participate in physician-led and multidisciplinary rounds, and communicate with patients, families and all disciplines involved in patient management in order to streamline and identify and solve potential problems.
• Maintain effective communication.
• Initiate and oversee all pre-op and post-op education, collaborating with nursing staff to provide a comprehensive teaching plan. Assess potential learning problems (language barrier, etc.).
• Initiate and collaborate in research protocols, journal articles, abstracts and patient education material.
• Screen potential donor calls; coordinate and organize all aspects related to donor/recipient status at time of transplant. 24 hour call responsibility for both donor call and outpatient problems. Triage calls, initiating appropriate treatment/intervention as required.

Pay Range
