Stony Brook Medicine is seeking a TH CTS Transplant Senior Specialist/Transplant Coordinator to provide comprehensive clinical services for our transplant patients. The role involves coordinating pre-transplant evaluations, education for patients and caregivers, and collaboration with a multidisciplinary team to ensure excellent patient care.

The position requires NYS RN licensure, strong communication, and experience in critical care.

Position Summary

At Stony Brook Medicine, a TH CTS Senior Transplant Specialist /Transplant Coordinator is a valuable member of our team, who provides clinical services to our patient population. Qualified candidates will demonstrate superior patient care and possess outstanding communication skills while adhering to our high standard of excellence.

Duties may include the following but are not limited to:

• Working with pre-transplant multi-disciplinary team to conduct new patient evaluations and re-evaluations of patients on annual basis.
• Coordinates patient entry and introduction to the transplant program.
• Coordinates pre-transplant education for patients and their caregivers.
• Works with the outreach coordinator to provide professional education for community nephrologists and dialysis center staff.
• Coordinates medical, surgical, financial, psychosocial, nutrition, and pharmaceutical evaluation and on-going care needs of pre transplant patients.
• Participates in patient care planning and selection conference.
• Lists patients for transplant in a timely manner to facilitate access to donated organs.
• Helps to maintain the UNOS waiting list according to program policies and procedures, regulatory requirements, and national guidelines.
• Remains knowledgeable of all listed patients\' status to ensure patient safety and facilitate organ placement in collaboration with transplant physicians.
• Works with multidisciplinary team on performance improvement projects.
• Assessment and Management of potential living donors.
• Coordinate the pre living donation education for patients.
• Coordinates the medical, psychosocial and financial evaluation of living donors.
• Attending meetings, national, regional, local and internal.

Qualifications

Required

• NYS licensed RN.
• Knowledge of Office/EXCEL.
• Excellent communication skills.
• Critical care/ICU experience as an RN.

Preferred

• CCTC (Certified Clinical Transplant Coordinator).
• Experience with ESRD/kidney transplant.

Pre-Start Requirements

• Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Medicine's Employee Health Services*.
• Complete electronic reference check with a minimum of three (3) professional references.
• Successfully complete a 4 panel drug screen*.
• Meet Regulatory Requirements for pre-employment screenings.
• Provide a copy of any required New York State license(s)/certificate(s).

Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.

• The hiring department will be responsible for any fee incurred for examination.

Anticipated Pay Range

The salary range (or hiring range) for this position is $98692 - $122712 / year.

The above salary range represents SBUH's good faith and reasonable estimate of the range of possible compensation at the time of posting. The specific salary offer will be based on the candidate's validated years of comparable experience. Any efforts to inflate or misrepresent experience are grounds for disqualification from the application process or termination of employment if hired.

Annual Supplemental Pay

• Location pay for UUP full-time positions ($4000)
