Job summary

Corewell Health's Transplant Admin - Grand Rapids is seeking a skilled registered nurse to coordinate complex heart, lung and kidney transplant processes. The role requires leadership in patient care, discharge planning, and collaboration with the multidisciplinary team to ensure optimal outcomes.

You will work within a comprehensive transplant program, supporting pre-transplant evaluations, intraoperative coordination, and post-transplant follow-up, while educating families and coordinating

Qualifications

• Bachelor's Degree in Nursing is required.
• Master's Degree in Nursing is preferred.
• Minimum 5 years of nursing experience with 2+ years in transplant or critical care.

Responsibilities

• Coordinate transplantation for all recipients across pre-, during-, and post-transplant stages.
• Provide follow-up and care throughout the transplant continuum with the care team.
• Educate patients, caregivers and hospital staff on transplant processes and post-transplant care.
• Maintain on-call coverage and coordinate with donor procurement and medical teams.
