Job summary

Harbor, Inc. is seeking a Transitional Support Specialist to provide personal care and support to clients in the TIP program. The role includes planning skill-building activities, helping with meals, supervising self-administration of medications, and assisting with daily living activities.

The position is part-time on the 2nd shift (24 hours per week) with set hours Thursday evening, Friday night, and Saturday night, including mandatory staff meetings and supervision sessions.

Qualifications

  • High School diploma or GED required; bachelor's in human services preferred.
  • Experience with severely mentally disabled population preferred.
  • CPR/First Aid and NCI/CPI certifications required within 90 days.

Responsibilities

  • Supervise and/or assist residents' daily living activities.
  • Supervise the self-administration of medication.
  • Assist or prepare client resident meals.
  • Follow care plans and provide observations to the clinical team.
  • Maintain accurate clinical records in a timely manner.
  • Ensure safety and infection control procedures are followed.
  • Organize recreational and educational activities.
  • Complete all tasks and cover shifts as needed.

Job description

Harbor is seeking a Transitional Support Specialist to provide personal care services or accommodations to clients enrolled in the Transition to Independence Process (TIP) program. This can include planning and leading skill development groups, assisting with meal preparation, supervising the self-administration of medication and assisting with activities of daily living.

Position is part-time 2nd shift, 24 hours per week working Thursday 2:45-11pm, Friday 10:45-7am, and Saturday 10:45-7am. Participation in the Thursday weekly staff meeting from 3p - 4:30p and a biweekly 1-hour supervision meeting are mandatory.

Education/Experience/Other Requirements:

  • High School Diploma or GED required. Bachelor degree in human service field preferred.
  • Experience working with severely mentally disabled population or similar population preferred. Experience working with severely mentally disabled young adult population or similar population preferred.
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.

Essential Job Competencies/Primary Duties:

  • Supervise and/or assist residents' with daily living activities.
  • Supervise the self-administration of medication.
  • Assist or prepare client resident meals.
  • Knowledge of the individualized care plan for residents and provision of support to the resident according to the care plan. Contribute to the care planning process by providing the lead clinical team with specific information and observations of the residents' needs and preferences.
  • Maintain accurate, complete and appropriate clinical records in a timely manner.
  • Perform all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.
  • Organize, develop and implement planned recreational/educational activities.
  • Responsible for the completion of all scheduled tasks, care of residents and the facility (including maintenance and cleaning).
  • Must be available for back-up and emergency shift coverage as needed. Able to work a flexible schedule.
  • Participates in training and development programs to enhance knowledge, skills and personal service networks.
  • Maintains courtesy and professionalism with staff and clients at all times.
  • Understands and maintains professional boundaries in client interactions.
  • Must complete trainings as specified by agency, licensing, funding and/or accreditation boards.
  • May float to other locations to accommodate client or agency need.
