Job summary

Rite of Passage at Walton Academy for Growth and Change in DeFuniak Springs, FL seeks a Transitional Services Coordinator to supervise a caseload of 14–24 students and assist with court hearings, probation visits, and family meetings.

You will serve as the primary liaison between students, families, placing agencies and staff; provide monthly progress notes and weekly updates, and coordinate activities with the treatment team.

Qualifications

• Have a bachelor's degree or better.
• Be able to pass a criminal background check, drug screening, physical, and TB Test.

Responsibilities

• Supervises a caseload of approximately 14-24 students.
• Assists with facilitating court hearings, Probation Officer visits, and student home passes.
• Primary program liaison between student, parents/guardian, and placing agency officials.
• Provides monthly progress notes and weekly student updates.
• Coordinates activities with other program staff and participates in assessments and treatment plans.

Rite of Passage Team is hiring for a Transitional Services Coordinator at Walton Academy for Growth and Change in DeFuniak Springs, Florida. Located near the white sand beaches of Northwest Florida in DeFuniak Springs - At Walton Academy, we provide residential treatment dedicated to improving the lives of at-risk male youth through mental health and substance abuse services. We offer skills and opportunities to our youth from Carpentry/Home Building Institute, Sports to Culinary Arts.

Transitional Services Coordinator

Compensation: $45,600 Annually

What you will do:

As a Transitional Services Coordinator, supervises a caseload of approximately 14-24 students. Assists with facilitating court hearings, Probation Officer, visits, student visits and/or home passes, and meeting with students to review progress. Primary program liaison between student, parents/legal guardian, and placing agency officials. Provides monthly progress notes and weekly student updates. Coordinates activities with other program staff. Participates in assessments and treatment plans. This position reports to the Director of Student Services.

Schedule

5 days a week to include a weekend day (Current schedule Sunday-Thursday).
