Job summary

Jobtailor is seeking an experienced Registered Nurse to perform comprehensive hospice assessments, educate patients and families, and coordinate care within a multidisciplinary team. The role emphasizes accurate data collection, DME ordering, and effective use of health tech like EPIC and HCHB.

The ideal candidate has an Associate Degree in Nursing and 2+ years of RN experience, including acute care or oncology; hospice experience is preferred.

• Two years of RN experience with acute care or oncology; hospice preferred.
• Proficiency with MS Office, EPIC, and HCHB is preferred.

Responsibilities

• Assess patient and family to determine hospice needs and plan care.
• Obtain demographic and medical history and communicate with clinical team.
• Order DME, supplies, and medications related to hospice care.
• Provide patient education on hospice, disease progression, and end-of-life care.
• Attend discharge planning and educate at bedside as requested.

Job description

Complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs
Obtain complete/accurate demographic information and medical history
Introduce the Agency to the patient/caregiver
Order requested DME, supplies, and medications related to hospice care
Deliver community education as requested
Provide patient education at bedside regarding hospice, disease progression, and end-of-life care
Attend discharge planning meeting as requested
Communicate with the clinical team on all complex, chronically ill patients

Requirements

• Associate degree from an accredited School of Nursing
• A minimum of two (2) years of experience working as a Registered Nurse including acute care or oncology experience, hospice experience preferred
• Experience with technologies such as Microsoft Office Suite, EPIC, and HCHB preferred
• Able to transfer patients as needed according to proper techniques of body mechanics to promote patient and staff safety
• Ability to accept different lifestyles, cultures, beliefs, and values
• Proficiency with computers
