Job summary

Jobtailor is seeking an experienced Registered Nurse to coordinate and deliver hospice care across an Illinois service area. You will lead nursing assessments, plan and monitor end-of-life care, and ensure compliance with Medicare and state regulations.

The role requires strong communication, empathy, and teamwork with the interdisciplinary team, patients, and families. On-call responsibilities include triage and on-site visits to support patients and families.

Qualifications

  • Licensed Registered Nurse with valid Illinois license
  • Diploma or degree in nursing from an approved program
  • Minimum 1 year of nursing experience in hospice or relevant field

Responsibilities

  • Coordinate and provide direct hospice patient care using the nursing process
  • Meet patients and families' physical, psychosocial, environmental, and spiritual needs across the geographic area
  • Interpret and follow hospice regulatory requirements, covered services, recertifications, transfers, and documentation standards
  • Perform case management through assessment, coordination, planning, monitoring, implementation, and evaluation
  • Assess patients, caregivers, and families; plan care; arrange services; monitor care; and provide support and education
  • Complete admission, recertification, visit, telephone, physician order, routine, supervisory, and discharge documentation accurately and on time
  • Coordinate the healthcare team and maintain linkages across the continuum of care
  • Communicate and initiate case conferences with other disciplines, physicians, care coordinators, and insurers
  • Direct licensed practical nurse activities and supervise home health aides
  • Assess patients for appropriate hospice placement under Medicare guidelines
  • Make home visits to monitor end-stage disease and end-of-life symptoms
  • Implement symptom-palliation interventions with attending physicians
  • Triage phone calls while on call
  • Visit patients to assess and pronounce death, perform post-mortem care, and notify the county coroner, funeral home, and physician
  • Provide family comfort and refer appropriately to the Bereavement Coordinator

Requirements
  • Licensed Registered Professional Nurse (RN) through the Illinois Department of Financial and Professional Regulation (IDFPR)
  • College diploma in Nursing
  • 1 year of nursing experience
