Job summary

Jobtailor is seeking an experienced Registered Nurse for hospice/home health care in California. The RN will manage caseloads, assess patient and family needs, and develop individualized care plans to ensure high-quality, compassionate care across the care continuum.

You will collaborate with physicians and an interdisciplinary team, oversee care delivered by Home Health Aides and volunteers, and participate in weekend/holiday on-call rotations.

Qualifications

• Graduate of an accredited school of nursing.
• 2 years experience as a RN.
• Preferred - Hospice or Home Health experience.
• Required within 30 days of hire: National Provider BLS - American Heart Association.
• Required upon hire: California Registered Nurse License.
