Job Title: Transitional Housing Program Coordinator

Reports to: Director of Housing

FLSA Code: Full time, Exempt with benefits

Hours: 40-hour workweek; flexible schedule that includes night and weekend work

Breakthrough offers a new model for homeless services. We are intentional about treating every person in need with dignity and respect, so they receive much more than a bed to sleep in—they receive the tools necessary to achieve self-sufficiency.

Breakthrough’s women and men's transitional housing shelter first aim to meet immediate needs of food and shelter through our kitchens, on-site laundry and showers. Guests then receive on-site primary and mental health care, employment training, and housing search assistance and placement.

Position Summary

The Program Coordinator works as part of a team and independently to provide assistance to individuals seeking homeless intervention services through Breakthrough’s programs. The Program Coordinator has basic responsibility for shelter shift coverage, crisis intervention and supervision of Program Assistants employed in the Men’s/Women’s Center.

Essential Responsibilities/Functions

  • Plan and organize evening programming through the trauma informed lens with individual restoration of guests as the goal
  • Supervise, manage, train and coordinate the activities of all assigned staff using approved performance management processes and policies
  • Oversee the monthly house meetings with guests
  • Ensure proper documentation and adherence to Breakthrough’s policy and procedures regarding guest interactions
  • Enter participant information into the ECM/HMIS database in a timely matter and update as needed in accordance with Breakthrough Urban Ministries established guidelines
  • Ensure data is collected and input into database properly, and make updates & corrections as needed
  • Prepare and submit reports to supervisor as required
  • Attend all required staff meetings and development sessions
  • Provide crisis intervention, safety planning, grievance resolution and resource information to guests
  • Meet with individual guests to support case management efforts
  • Assist with developing contracts for guests with positive alcohol and/or drug testing results
  • Work with Behavioral Health Coordinator to plan, lead and organize SA groups including NA/AA
  • Perform other duties as assigned

Qualifications

  • Minimum of two years supervisory experience
  • At least one year of directly related experience or minimum of 3 years of customer service experience
  • Counseling or life coaching experience desired
  • Should have experience working with individuals dual-diagnosed
  • Must possess an accurate and informed understanding of cultural and urban realities, especially as related to the homeless population
  • Knowledge of standard case management protocols
  • Experience with Microsoft Office, as well as the willingness to become proficient in other software packages and programs
  • Good interpersonal, oral, and written communication skills
  • Prepare accurate reports.
  • Must possess an accurate and informed understanding of cultural and urban realities, especially as related to youth, families, poverty, and community development
  • Must have a valid driver’s license

Physical Requirements And Working Conditions

Work Environment

While performing the duties of this job, the employee will work in a multi-purpose facility and/or outdoors and thus will be exposed to weather conditions prevalent at the time. The noise level in the work environment is usually moderate but can be high depending on the activities in the facility.

Physical Demands

While performing the duties of this job, the employee is frequently required to walk and/or sit; use hands and fingers to feel objects, tools or controls, reach with hands and arms; balance; stoop; regularly talk or hear. The employee must be able to bend, stoop, pull, lift as well as have overhead extension of arms. Must be able to stand for long periods of time; be able to lift, carry or move at least 50 pounds. Must adhere to proper handling and lifting techniques. Specific vision abilities required by the job include close vision, distance vision, peripheral vision, depth perception and the ability to adjust focus.

Equipment Used

The employee will generally use general office equipment including but not limited to telephone, computer, calculator, copier, overhead projector, paper shredder, paper cutter, and filing systems.

  • 401 (k) Plan - We offer a 401(k) plan which is a tax-deferred annuity to assist full-time employees in planning for retirement. We match 50% of your contribution up to a total of 3% of your base salary.
  • Section 125 Plan (FSA) - Through TASC, we allow full-time employees to make contributions toward premiums for medical insurance, out-of-pocket medical expenses, dependent care expenses, and/or transit and parking on a “before tax” rather than an “after tax” basis.
  • Vacation Days - Full-time employees in this position are eligible to accrue vacation days at a rate of 6.67 – 13.33 days per month based on tenure with the organization.
  • Personal Days - Full-time employees in this position are eligible for up to 3 personal days annually, pro-rated in year one.
