Job Details: Job Location: NEW DIRECTIONS Family Service Center - Oceanside, CA 92057, Position Type: Full Time, Salary Range: $70,304.00 - $80,000.00 Salary/year, The New Directions Intake Coordinator is responsible for leading outreach, referral screening, intake coordination and assessment for the Transitional Housing Programs. This position supports census achievement and program growth through culturally responsive, trauma-informed engagement practices and proactive community outreach. The Intake Coordinator also facilitates internal service alignment, assists with housing coordination, and contributes to the overall functioning and quality improvement of the program. If clinically qualified, the Intake Coordinator may provide direct clinical services including but not limited to assessments, interventions, treatment planning and care coordination to support the stability and well-being of Casa de Amparo's clients regardless of payer or funding stream. The majority of responsibilities for this position require advanced professional knowledge, independent clinical judgment, and the application of evidence-based practices in both direct client care and community partnership development.

Outreach, Referral Development & Intake Coordination
  • Promote the transitional housing programs through strategic outreach and relationship- building with referral sources, community partners, and public systems to support awareness and census.
  • Develop and maintain professional collaboration with County representatives, federal representatives, community partners to enhance program visibility, achieve and maintain target census, and ensure coordinated client care.
  • Collaborate with the development team to identify marketing needs and implement strategies that enhance visibility of the transitional housing program.
  • Establish and maintain collaborative relationships with ECM providers, health systems, and other service organizations to strengthen care coordination.
  • Support ECM enrollment and service coordination, including documentation and collaboration with external providers.
  • Serve as a central point of contact for Casa de Amparo’s housing referrals.
Assessment & Clinical Service Coordination
  • Build rapport using trauma informed, culturally responsive, harm reduction, and youth centered approaches.
  • Conduct comprehensive, trauma informed screenings and interviews of potential participants and collateral references to assess eligibility, identify needs, and determine program fit.
  • Complete clinical assessments and use independent professional judgment to determine eligibility, clinical needs, risk factors, and appropriate referrals (ECM, outpatient mental health, community supports). Provide crisis intervention and problemsolving support during initial engagement.
  • Screen for indicators of trafficking, exploitation, or acute safety concerns and respond appropriately.
  • Facilitate warm handoffs to emergency shelters, crisis services, ECM, behavioral health providers, and community partners. Assist participants in accessing educational, employment, health, mental health, and financial resources to support housing stabilization.
  • Coordinate onboarding and orientation by scheduling intakes, assigning new participants to an apartment and to an Advocate's caseload, in collaboration with transitional housing leadership.
  • Collaborate with program staff to ensure smooth participant transitions into services.
  • Ensure all orientation packet materials; required vital documents and the Health and Education Passport (HEP-if applicable) are completed in full and submitted to assigned Advocate at least three days prior to movein.
  • Oversee the timely completion of all placement documentation and ensure county representatives are fully informed with key movein details, including address, assigned advocate, and other essential information.
  • Monitor eligibility/waitlists and census trends; follow up with clients and coordinate next steps in the housing process.
  • Ensure timely and accurate census data is entered into all required databases and documents.
  • Provide crisis intervention services as needed, utilizing evidence-based practices and exercising clinical discretion in treatment planning and implementation.
  • Participate in the transitional housing programs oncall rotation to provide afterhours support, crisis response, and service coordination.
Housing Coordination & Support
  • Serve as a liaison to housing providers, shelters, apartment complexes, and other community-based resources.
  • Assist with transitional housing unit acquisition and coordination of unit readiness, including furnishings, utilities, and supplies, in collaboration with the Housing Operations Manager.
  • Provide support with participant moves and assist with unit cleaning or transition-related needs.
  • Assist in managing program inventory in alignment with the program budget and agency policies.
  • Participate in monthly and quarterly housing inspections to ensure units meet program standards.
Documentation, Reporting & Compliance
  • Maintain timely and accurate data entry, documentation, and outcome reporting in accordance with internal policies and funding/regulatory requirements for both housing programs.
  • Maintain waitlists in accordance with policies and procedures.
  • Ensure ongoing compliance with HIPAA, contract guidelines, Community Care Licensing regulations, Joint Commission accreditation standards, and agency procedures.
Program & Volunteer Support
  • Support the effective integration of volunteer resources into program where appropriate.
  • Perform other duties as assigned in alignment with program goals and agency mission.
Qualifications
  • Must be able to be associated with Casa de Amparo through Community Care Licensing (CCL), which typically includes clearance by FBI, DOJ and CACI.
  • Bachelor's degree in social work, psychology, human services, or related field, Master's degree preferred.
  • Licensed (LCSW, LMFT, LPCC) or license-eligible in California with current registration in good standing (e.g., Associate registration with the BBS) preferred.
  • Minimum of two years' experience in a public or private social services setting preferred.
  • Experience in social services, Transitional Age Youth (TAY), or foster youth preferred.
  • Strong understanding of TAY development, the effects of trauma, California Title 22 regulations and San Diego County private/public social service agencies.
  • Well-organized, strong telephone and interpersonal communication and interviewing skills, detail and big picture oriented.
  • Ability to comply with all employment requirements including, but not limited to drive agency vehicles with clear DMV report, reliable transportation with auto insurance.
  • Must provide proof of required immunizations.
  • Must be able to perform the essential functions of the job, including lifting up to 50 lbs, crouching, crawling on hands and knees, sitting, and climbing stairs, with or without reasonable accommodation.
  • CPR/1st Aid certificate required.
  • Must pass a pre-employment drug screen, health exam and TB test.
  • Must be at least 21 years of age.
