Job summary

A leading healthcare provider in New York is seeking a Transitional Care Nurse to ensure comprehensive post-discharge care for patients. The role requires an RN licensure in New York, 2 years of relevant experience, and a Bachelor's in Nursing. The nurse will work closely with an interdisciplinary team and community agencies to prevent patient readmissions. Ideal candidates should be proficient in BLS and ACLS, and hold an MSN as a preference.

Qualifications

• Must have current RN licensure in New York State.
• 2 years of Med-Surg, ED & Community Health experience required.
• BSN and BLS & ACLS certifications are mandatory.

Responsibilities

• Ensure post-discharge collaboration for patient needs.
• Coordinate comprehensive care available for patients after hospital discharge.
• Work as part of the interdisciplinary team.

Description

The Transitional Care Nurse works as a part of the interdisciplinary team, as well as with community agencies, to ensure post-discharge collaboration in meeting patient's needs as determined by the social determinants of health screening. The TCN ensures that comprehensive care in the community is available for the patient in the post-hospital discharge period, with the goal of preventing readmission.

Requirements:

• Current RN licensure in New York State
• 2 years of Med-Surg, ED & Community Health experience required.
• BSN required
• MSN preferred
• BLS & ACLS
