Job summary

Butler County Housing Support Services/CRR in Butler, PA is seeking a compassionate professional to support residents with mental health needs transitioning from incarceration into housing, offering a choice between custody and independence.

You will provide hands-on instruction, assist with daily living activities, manage medications and budgeting, help with job placement, document progress, and participate in training to respond to crises while maintaining strict confidentiality.

Qualifications

• High school diploma or equivalent required; may consider experience.
• Valid PA driver's license with safe driving record.
• Ability to communicate information effectively.
• Experience with electronic medical records.
• Ability to transport clients in insured vehicle.
• Criminal history and FBI clearances required.
• Ability to work in crisis and support residents.

Responsibilities

• Provides hands-on instruction to residents on implementing Individual crisis plans and assisting with crisis resolution.
• Help residents live as independently as possible and reach attainable goals to maintain independence.
• Aids and support with Activities of Daily Living and Instrumental ADLs per assessment and support plan.
• Assist with meal preparation, household chores, medication management, budgeting, and accessing community facilities.
• Assist with skill development and job placement.
• Monitor residents' progress and complete documentation, identifying issues to address.
• Participate in staff training, including crisis-resolution and job performance training.
• Respond to emergency situations appropriately.
• Maintain detailed documentation of all assistance provided to residents.
• Other duties as required; adhere to policies and confidentiality.
