Qualifications

• Bachelor's Degree BSN is required.
• Minimum 3 years of nursing experience in an acute care setting is required.
• Experience in a clinical healthcare setting or insurance setting responsible for direct management of patient care; including planning, coordination, discharge planning and delivery of needed services is preferred.

Responsibilities

• Facilitates the patient's progress through the acute episode of care and creates a transition plan from inpatient to post-acute settings.
• Initiates evidence-based protocols and order sets according to admission diagnosis, influencing care progression.
• Advocates for the patient and family and coordinates communication with the multidisciplinary team, referring physician, and ambulatory care team.
