Job summary

MCS Health, a division of MCS Healthcare Holdings, LLC, seeks a registered nurse to manage discharged insureds in the Transitional Care program within the Classicare line. This role centers on coordinating care plans, reviewing treatments, and ensuring timely follow-up for patients transitioning from hospital discharge.

Responsibilities include coordinating services with clinical units, assessing patients for needs, and ensuring adherence to Medicare guidelines.

Qualifications

• Bachelor's degree in Nursing Sciences with at least three years of experience in clinical areas and documenting or analyzing medical records.

Responsibilities

• Identify discharged insureds who apply to the transitional care program and assess needs within the established period after discharge.
• Establish a care plan with goals, coordinate visits, review medications, and identify a contact person for 30 days in the program.
• Conduct follow-up interventions by telephone according to risk level and close cases within the established period.
• Coordinate referrals with Clinical Units to implement services and evaluate need for discharge coordination services.
