Job summary

Carle Health in Urbana, Illinois is seeking a Nurse Practitioner to join the Transitional Care Clinic (TCC) at Carle Foundation Hospital. Qualifications

• Master’s degree in nursing required or preferred for APN practice in primary care/hospital settings.
• Two years of APN experience in primary care or hospital setting preferred.
• Current Illinois RN license and Advanced Practice Nurse licensure.
• BLS and APN national certification.

Responsibilities

• Perform health assessments for return-to-wellness and determine intervention needs.
• Ensure documentation of care is complete and accurate.
• Communicate with health professionals, families and referral agencies about patient status.
• Lead nursing practice improvements and supervise patient care delivery by support staff.
• Assess patients referred to TCC and determine eligibility.
• Place scheduling orders for TCC patients and provide ongoing care for 30 days post-hospitalization.
• Place follow-up orders and monitor patient progress.
• Coordinate return of patient care to the Primary Care Provider.
• Engage in case management and conduct home visits within a 60-mile radius.
