Job summary

One Health is seeking a Nurse Practitioner or Physician Assistant to join the Transitional Care Management team. This role focuses on caring for high-risk patients recently discharged from the hospital, aiming to improve outcomes and reduce readmissions.

The candidate will collaborate with hospitalists, ED providers, and case management to ensure a smooth transition back to the community, within an evidence-based, patient-centered care model.

Qualifications

• Board Certified/Board Eligible Nurse Practitioners and Physician Assistants are welcome.

Responsibilities

• Provide Transitional Care Management services to recently discharged patients.
• Collaborate with hospitalists, ED providers, and Case Management teams to coordinate post-discharge care.
• Work within a multidisciplinary team including RNs and other care professionals.
• Support patient-centered transitions to the community and improve outcomes.
