Job summary

Independence Home Health LLC is seeking a compassionate Transitional Aide to support clients through transitions in care, at home. The role requires reliability, flexibility to cover shifts, including evenings and weekends, and a dedication to patient wellbeing.

You will assist with activities of daily living, follow care plans, and collaborate with supervisors and the care team to ensure safety, comfort, and independence for clients.

NOW HIRING: TRANSITIONAL AIDE

Independence Home Health LLC

We are seeking a compassionate, dependable, and flexible Transitional Aide to join our team! If you enjoy helping others and can adapt to changing schedules and client needs, we would love to hear from you.

Pay: $18.00 per hour

Guaranteed 40 hours per week

REQUIREMENTS

• This position requires exceptional scheduling flexibility and availability around the clock (24/7) to help cover unexpected staff absences, accommodate new client needs, and fill shifts on short notice, including evenings, weekends, and occasional overnights.
• Reliable transportation.
• Ability to assist clients during transitional periods of care.
• Strong communication and time-management skills.
• Dependable, professional, and compassionate demeanor.
• Ability to follow care plans, agency policies, and instructions from supervisors.

KEY RESPONSIBILITIES

• Provide support and assistance to clients during transitions in care.
• Assist with activities of daily living as needed and within the scope of the role.
• Follow established care plans and agency policies and procedures.
• Communicate effectively with supervisors, caregivers, and other members of the care team.
• Help promote client comfort, safety, and independence.
• Adapt to changing client needs and scheduling requirements.
