Job summary

Hamden Public Schools in Hamden, CT is seeking a Transition Support staff member to help high school students with disabilities prepare for postsecondary education, employment, independent living, and community participation. The role provides direct instruction, career development, and employment support through individualized and small-group activities.

You will collaborate with families, educators, and community partners, develop transition goals aligned with IEPs, and coordinate programs

Qualifications

  • Experience working with adolescents, individuals with disabilities, or transition-age youth.

Responsibilities

  • Provide direct transition instruction in individual and small-group settings.
  • Facilitate lessons and activities focused on self-advocacy, independent living, financial literacy, career exploration, workplace readiness, communication skills, and postsecondary planning.
  • Support students in developing and achieving individualized transition goals aligned with their IEPs.
  • Monitor student progress and collaborate with special education staff regarding student growth.
  • Prepare students for employment through instruction in job applications, resumes, interviewing, workplace expectations, and employment skills.
  • Coordinate and support job shadowing, volunteer experiences, community-based instruction, and work-based learning opportunities.
  • Develop relationships with local employers and community agencies to expand student employment opportunities.
  • Support students during employment placements and provide coaching to promote successful work experiences.
  • Coordinate and oversee the Hamden Public Schools Work Study Program.
  • Recruit, place, and support student employees at district worksites.
  • Conduct worksite visits and collaborate with supervisors to ensure successful student employment experiences.
  • Monitor student participation and communicate regularly with students, families, school staff, and employers.
  • Serve as the district liaison with community agencies, including the Bureau of Rehabilitation Services (BRS), Level Up, Junior Achievement, and other transition partners.

Position Summary

Student Transition Services
