Job summary

Paycom - ATS is seeking a Transition Specialist in Chesapeake, VA to provide education, release planning, case management, and re-entry preparation for clients nearing release from the Indian Creek Correctional Center. The role focuses on arranging continuing care and facilitating re-entry activities with community partners.

The position requires a Bachelor's in Human Services and Virginia CSAC certification within two years, plus experience in case management and substance abuse treatment.

Qualifications

• Bachelor's Degree in Human Services required.
• State certification as a Substance Abuse Counselor (CSAC) in Virginia or equivalent within two years of signing.
• Case management experience required.
• Experience in substance abuse treatment preferred.
• Experience in a correctional or secure environment is a plus.

Responsibilities

• Provide group education, release planning, case management, and re-entry preparation to clients who completed the Cognitive Therapeutic Community program and are within 90 days of release.
• Arrange continuing care needs for returning citizens to prepare them for treatment, support, and living needs after release.
• Facilitate Thinking for a Change classes, attend Re-Entry Council meetings in Tidewater, and coordinate workshops with external vendors.
• Collaborate with Virginia Department of Corrections personnel to ensure quality services and promote a healing recovery environment.

Indian Creek Correctional Center is a level 2 medium security prison. This facility provides specialized treatment services to individuals with substance use disorder

Hours: Monday – Friday, 7:00 am – 4:00 pm OR 7:45 am – 4:45 pm with a 1-hour unpaid lunch break

Pay Rate: $25.00/hour. ($52,000 annually). Candidates with a CSAC, CADC, or equivalent certification/license are eligible for $27.88/hour. ($58,000 annually)

The Transition Specialist is responsible for:

• Providing group education, release planning, case management, and re-entry preparation services to the clients who have completed the Cognitive Therapeutic Community program at Indian Creek Correctional Center and are within 90 days of their release from incarceration
• Arranging continuing care needs for the returning citizens to prepare them to meet their various treatment, support, and living needs once they are released from incarceration
• Facilitates Thinking for a Change classes, attends Re-Entry Council meetings in the Tidewater area, and sponsors outside vendors to conduct workshops and seminars for the returning citizens
• Working closely with Virginia Department of Corrections personnel to ensure quality services and promote a healing environment where people with substance use disorders can learn to live in recovery as returning citizens

Qualifications

• Bachelor's Degree in Human Services field is required
• State certification as a Substance Abuse Counselor (CSAC) in the state of Virginia or similar certification/license recognized by the Virginia Board of Professional Counselors is required within two (2) years of signing job description
• Case management experience is required
• Experience working in the substance abuse treatment field is preferred
• Experience working within a correctional or secure environment is a plus
• Experience working in Therapeutic Community model programs is a plus
