Job summary

Qualifications

• Special Education Certification and/or pertinent experience with special education, secondary transition, and preparing students with disabilities for employment.
• Supervisory experience preferred.
• Strong collaboration and communication skills.
• Customer service strength across a variety of stakeholders.
• Successful completion of all pre-employment requirements.

Responsibilities

• Coordinates transition planning with families, classroom teams, and agencies for seamless student transitions.
• Develops professional development content and parent workshops to improve post-school outcomes.
• Serves as embedded coach/mentor, ensuring staff stay current in evidence-based transition practices.
• Ensures PDE/ODP guidelines are met and tools related to transition are up to date.
• Oversees school career preparation services with an employment-first approach and manages related billing.
• Acts as secondary transition lead across school districts, parents, agencies, and other stakeholders.
• Coaches team members and addresses performance through feedback and training.
• Manages department timeline and budgetary needs.
• Addresses performance discrepancies and training gaps as needed.
• Implements performance management practices to achieve high performance.
• Completes other duties as assigned.
