Job summary

The Davenport Community Schools is seeking a Transition Alliance Program (TAP) Coordinator to lead transition services for high school students and young adults with disabilities at West High School.

You will coordinate counseling, assessments, and job development, and collaborate with IVRS and providers to create transition plans aligned with IEPs. The role requires maintaining records and guiding students toward competitive employment through age 25.

Qualifications

• Strong interpersonal skills with a proven ability to build rapport with culturally diverse students.
• Valid driver’s license required.
• Excellent oral and written communication skills.
• Self-motivated, flexible and able to meet deadlines.
• Computerskills necessary to perform job searches and assist students online.

Responsibilities

• Plan and coordinate transition services for high school students and young adults with disabilities.
• Develop transitions plans in coordination with IEPs and Employment Plans.
• Collaborate with IVRS, school personnel and adult service providers.
• Maintain case records and reports and provide follow-up until age 25.
• Supervise and support TAP-related activities and outreach.
