Seventh Dimension LLC is seeking a Transition Nurse Case Manager (RN) to support Naval Special Warfare Service Members transitioning from military service in Coronado, CA. The role focuses on reviewing medical needs, coordinating continuity of care, facilitating referrals, supporting healthcare planning, and connecting Service Members with resources.

Qualificações

• US Citizen
• Active Secret security clearance
• RN license in a US state, DC, or US territory
• Two years of professional nursing experience
• Case-management certification (CCM/ACMA/ANCC) or ability to obtain within one year
• Proficiency with Microsoft Office (Word, Outlook, Excel, PowerPoint)

Responsabilidades

• Provide professional nursing and case management support to transitioning service members.
• Collect, organize, document, and communicate health assessment and care coordination information.
• Develop individualized care plans with milestones, responsibilities, and care coordination actions.
• Conduct authorized telephonic and virtual case management assessments.
• Establish priorities for case management support and monitor progress toward identified goals.
• Coordinate referrals to healthcare providers, community resources, and authorized support services.
• Collaborate with military, civilian, veteran, and community healthcare providers to support continuity of care.
• Provide health education to Service Members and family members within the nurse's scope of practice.
• Identify quality, utilization, safety, and risk management concerns and report them as required.
• Maintain case management documentation, workload data, metrics, and reports.
• Participate in case reviews, meetings, training events, and coordination activities.
• Assist transitioning Service Members with understanding healthcare transition options and VA resources.
