Job summary

A prominent community health organization in Cincinnati is looking for Case Managers / Youth Transition Facilitators to provide vital support to young adults with severe mental illness. The ideal candidates will possess a Master's degree in mental health or a related field and have a passion for helping others. Qualifications

Overview

GCBHS has immediate openings for Case Managers / Youth Transition Facilitators to provide Community Psychiatric Support Treatment (CPST) services to severely and persistently mentally ill youth and young adults. CPST services are designed to provide specific, measurable, and individualized services to each person served. This is a great starting point for recent Social Work or Psychology graduates pursuing MSW or Master’s in Mental Health Counseling. Responsibilities
  • Provide case management – CPST and Therapeutic Behavioral Services (TBS) to youths and young adults with severe mental illness in outpatient and community settings
  • Initiate, coordinate, and monitor evaluations to identify client needs across living arrangements, financial issues, benefit status, legal status, health, activities of daily living, social supports, and recreation
  • Engage clients through assertive outreach to identify needs, offer services, and reassess outcomes regularly
  • Develop knowledge of mental health treatment resources, agency linkages, and costs of services; identify opportunities for alternative resources
  • Understand the population served and the practice setting, including socioeconomic, cultural, gender, racial, and sexual orientation considerations and their impact on service use
Qualifications
  • Masters degree in mental health, social work, counseling, education, or related field
  • OR BSW (Bachelor of Social Work) or Bachelor in psychology/criminal justice/related human services field with two years of specialized education, training, or experience in the specified fields
  • Experience in social services and behavioral health preferred but not required
  • Position requires frequent driving of own vehicle; valid driver’s license, vehicle, and insurable driving record required
