Job summary

Greater Cincinnati Behavioral Health Services is seeking a Care Management Team Manager to lead a team serving adults with severe mental health disorders in Hamilton County. The role emphasizes leadership, supervision, and coordination of outreach services to ensure high-quality care.

The position requires demonstrated supervisory skills and a Master’s degree in Counseling or Social Work (or progress toward one). Ohio licensure is preferred.

Qualifications

• Master's degree in Counseling or Social Work (or in progress) preferred.
• Demonstrated leadership and supervision experience required.
• Licensed by the Ohio Counselor, Social Worker and Marriage and Family Therapist Board - LSW or LPC preferred.

Responsibilities

• Participates in planning, design and development of services and marketing of services to the homeless system and other community representatives.
• Coordinates the activities and operation of the team, including client care collaboration with community services to ensure quality services.
• Provides leadership and supervision to team staff; facilitates regular team meetings and supervision meetings; identifies training needs.
• Hires, orients, trains, schedules and evaluates staff.
• Responsible for team outcomes and for collecting and managing data for outreach reporting.
• Conducts community presentations and trainings.
• Monitors outreach phone log and all team service documentation to ensure accurate, timely documentation.
• Provides and coordinates outreach services to clients and leads staff in outreach initiatives.

Position Responsibilities:

• Participates in planning, design and development of services and marketing of services to the homeless system and other community representatives.

• Provides leadership and supervision to team staff, including facilitating regular team meetings, documented one-on-one individual supervision meetings, identifying and addressing performance objectives and training needs, etc.

• Responsible for own and the team's outcomes. Responsible for collecting and managing data requested for outreach reporting.
• Conducts community presentations and trainings.
• Monitors outreach phone log and all team service documentation to ensure the completion of accurate and timely documentation of services provided. Ensures follow up takes place to stakeholders who contact the outreach team.
• Provides and coordinates outreach services to clients, including documentation of services provided.
• Provides leadership and direction to staff in areas of outreach services to clients

Position Requirements

• Licensed by the Ohio Counselor, Social Worker and Marriage and Family Therapist Board - LSW or LPC is preferred
• SW or Counselor Master's program can be considered
• Demonstrated leadership and supervision skills preferred.
