The Office of Juvenile Justice is seeking a dedicated Transition Coordinator to support educational continuity and successful re-entry for youth transitioning between community schools and secure-care settings. This role is ideal for a professional who communicates clearly, collaborates effectively, and values structured, student-centered planning.
As a Transition Coordinator, you’ll help develop individualized academic plans, maintain communication with schools and families, and support youth as they move into secure care and back into community education environments. You’ll work closely with case managers, educational staff, and community partners to ensure students receive the guidance, resources, and follow-up needed for long-term success.

To coordinate educational transitions for youth entering, residing in, and exiting secure-care programs; to serve as liaison between OJJ educational staff, case managers, community schools, and other stakeholders; and to ensure continuity of academic services and compliance with individualized transition planning requirements.
Level of Work: Programmatic / Educational Support
Supervision Received: General direction from the Director of Education or designated supervisory staff.
Job Distinctions: This position is responsible for coordinating academic transitions for youth assigned to secure care and returning to community schools or post-secondary pathways. Work involves collaboration with multidisciplinary teams, community schools, families, and OJJ staff to ensure continuity of educational services and compliance with statutory transition-planning requirements.
The Transition Coordinator supports students in successfully reintegrating into schools, communities, and vocational programs.
Work Location: This vacancy is located at Columbia Center for Youth in Columbia, Louisiana.
Appointment Type: This is a part-time Transition Coordinator position.
Compensation: The salary offered will be determined based on experience.
The incumbent of this position must not have been convicted or pled nolo contendere to a crime listed in R.S. 15:587.1 (c).
