SEEM Collaborative’s SEEM Prep in Woburn, MA is hiring a Transition Coordinator for the 26-27 school year. The role reports to the Program Principal and directs a team of job coaches, overseeing vocational training and transition activities for students with disabilities.

Responsibilities

Job Description

TITLE: Transition Coordinator

REPORTS TO : Program Principal, Assistant Principal, Director of Transition Services, Executive Director

QUALIFICATIONS:

Required:

  • Master's degree
  • DESE licensure in education (i.e. Moderate Disabilities, 5-12, Severe-All Levels) or similar certification in a related vocational/transition field.
  • A valid MA driver's license and safe driving record.

Preferred:

  • Experience working with high school and transition age young adults with exceptional needs
  • DESE Transition Specialist Endorsement

ESSENTIAL SKILLS:

  • Must demonstrate strong organizational skills; Must be able to work effectively with interruptions and reprioritize tasks as unexpected circumstances arise.

  • Demonstrate ability to use Microsoft Office, G-Suite and other district technology effectively
  • Must have mobility required to sufficiently assist students with disabilities with physical needs, personal care, and behavioral challenges. Must possess the ability to lift, push, pull and/or carry up to 40 pounds on an occasional basis and have the ability to move quickly from standing to floor and floor to stand. May be required to physically restrain individuals in situations where there is a risk of serious and imminent harm.

SCOPE OF DUTIES:

  • Support school mission and vision
  • Collaborate with principal, assistant principal, curriculum coordinator, clinicians, teachers, and consultants to identify areas to implement new and existing curricula, make recommendations for program improvement or initiatives
  • Collaborate with principal, assistant principal, curriculum coordinator and other consultants to implement school objectives and provide support for teachers to improve instructional practices
  • Assist in developing a culture that encourages lifelong learning, reflective practice and collaboration
  • Coordinate vocational training program including the development and operation of school and community-based work experiences, serving as the primary liaison between the school and worksites.
  • Coordinate van use for work and community trips. Oversee vehicle maintenance.
  • Lead transition department including ongoing supervision and training of job coaches
  • Guide development, implementation, in-service training programs for transition department
  • Complete Transition Planning Forms, Summary of Student Performance and collaborate with the IEP Team to create IEP objectives calculated to meet the students’ postsecondary goals.
  • Participate in Team meetings (three-year re-evaluations, annuals, progress/update, transition meetings) as needed.
  • Provide consultation to classroom teachers to ensure community trips are centered on students’ IEP and TPF goals.
  • Plan and maintain a transition curriculum to be implemented through the classrooms with the support of classroom teachers and instructional aides/job coaches.
  • Administer vocational and transition assessments for specific students (i.e. three year reevaluations, as requested by district) to inform the IEP and team process.
  • Provide direct training in the areas of independent living, community access and lifelong learning, and is responsible for assessing student needs and coordinating training and services within the individual’s community.
  • Coordinate with districts, state agencies, local service providers and SEEM Clinicians in regards to the status of Chapter 688 referrals and to ensure seamless transitions to adult services and supports.
  • Coordinate student participation in the Massachusetts Inclusive Concurrent Enrollment Initiative programs.
  • Assist teachers in the development of transition portfolios and assist and advise teachers on the implementation of MyCAP activities
  • Will work with school administration to collect Indicator 14 data on post-school outcomes.
  • Provide classroom coverage when needed. May be asked to cover for Teacher leave of absences.
  • Perform other tasks/responsibilities as assigned by the principal/assistant principal
  • Employment is contingent upon passing a SAFIS fingerprint and CORI screening.
  • Other duties as assigned by the Program Principal.

Position Type: Full-Time

Salary: $64,299 to $101,744 Per Year
